What is Social Prescribing?
 

Social Prescribing is a free programme available to anyone over the age of 18 years who
would like to improve their quality of life. The aim of Social Prescribing is to link people
into non-medical sources of support within their community so as to help them to
improve and take greater control of their health and wellbeing.

 

Who is Social Prescribing for?
 

The programme is particularly powerful for those who are lonely, socially isolated, managing

a chronic illness and/or living with mild or long-term mental health challenges such as

anxiety, stress or depression.

The service is suitable for adults who are wanting change and want to make a positive step

forward with support. For the best results, it is important that those who are referred to the

service are willing participants and are positive about improving their health and wellbeing.

What will our Social Prescriber do?
 

Our Social Prescriber aims to bridge the gap between you and the service/ support you

need. We will spend time talking about you and what matters to you. Your priorities might be

to meet new people, learn a new skill, take up exercise, access local supports or further your

education. Each individual plan is client led and our Social Prescriber facilitates access to

support services or community connections identified by the person they are working with.

 

Examples of supports and services available
 

Social groups: e.g. Men’s Shed, Community Garden, Women’s Group

Support groups: e.g. Mental Health, Domestic Violence, Therapeutic Support

Physical activities: e.g. a Walking Group, Zuma Class

Hobbies: e.g. an Arts and Craft Group

How do I make a referral?
 

Social Prescribing can be accessed by self-referring or being referred by your GP or any

other health and community professional by either contacting your local Social Prescriber or

completing a referral form attached and sending it to the following;
